2012 Registration Forms baseball and t-ball
Registering for T Ball is easy. REGISTER CLOSES APRIL 13th SO REGISTER TODAY.
1. Download a copies of both the Registration form and Parent Player Contract. (Forms can be filled out on line before downloading.)
2. Sign forms. Both player and parent or guardian sign the Parent Player Contract.
3. Mail, email or fax forms. (Check must be mailed.)
Be sure to include a photo copy of the players birth certificate and a check or money order for $75 to cover the cost of registration and uniform. (please identify players name when sending check).
